ArchWell Health is building-out in Gateway Town Center in Brentwood, the fifth area location for the primary care provider for seniors.
Hoar Construction LLC of Orlando is the contractor for all four in Duval County. Earl Swensson Associates Inc. of Nashville, Tennessee, is the architect.
The Nashville-based medical system focuses on senior care.
In Brentwood, the city issued a permit March 10 for Hoar Construction to build-out leased space at 5000 Norwood Ave., Suites 4-5, at a project cost of $3.37 million.
ArchWell will build-out 11,795 square feet of its 13,553-square-foot leased space, with the unused portion designated for growth.
ArchWellHealth.com lists the five area locations as coming soon.
The other four comprise:
• In West Jacksonville, the city issued a permit Aug. 28 for the clinic to build-out in West Side Plaza at a project cost of $3.025 million. ArchWell Health is building-out 8,740 square feet of space at 5930-34 Normandy Blvd. in the shopping center at southeast Normandy Boulevard and Lane Avenue.
• In Arlington, the city issued a permit Sept. 16 for the clinic to build-out an 8,250-square-foot building at 1072 Arlington Road N. at a project cost of $2.77 million. The location is north of the Arlington Expressway.
• In Southwest Jacksonville, the city issued a permit Oct. 17 for the clinic to build-out space at 7628 103rd St. in Oak Hill Village at a project cost of almost $2.65 million. ArchWell is building out 6,873 square feet of space for a health clinic at southwest 103rd Street and Ricker Road.
The West Side Plaza and Arlington properties are owned by Cardinal Management of Mattituck, New York.
• In Orange Park in Clay County, ArchWell Health intends to open in Blanding Plaza at 311 Blanding Blvd., Suite 4. The site is 2 miles south of Interstate 295. A build-out permit issued Oct. 2 for a $2 million project cost shows a size of 14,624 square feet.

The ArchWellHealth.com site says members will have appointments averaging 40 minutes; telehealth appointments; on-site lab work; 24/7 access to providers via phone or text message; comprehensive care on-site; access to health records via the online member portal; and a health specialist referral program.

ArchWell Health shows on its ArchWellHealth.com site that it has 54 locations open in nine states, including 10 clinics in Florida along the state’s Gulf Coast.
The 10 locations listed as coming soon comprise the five in Northeast Florida, another in Arizona and four that will open in Colorado as the 10th state of operation.
That raises the total to 64 in 10 states.
