The city issued a permit Dec. 22 for a $2 million tenant build-out for Conviva Care Center at 11240 Beach Blvd. in St. Johns Square, Phase 1.
Rycon Construction Inc. of Sunrise is the contractor for the 9,015-square-foot project. Gensler is the architect.
The location is next to Bailey’s Health & Fitness. St. Johns Square is at southwest Beach Boulevard and St. Johns Bluff Road.
The tenant is shown on site plans as Humana and the owner is listed as Humana Provider Holdings LLC.
Conviva, a provider of senior-focused primary health care in Florida and Texas, is a wholly owned subsidiary of Humana Inc.
The convivacarecenters.com site shows five locations in Jacksonville in North Jacksonville, Gateway, West Jacksonville, Arlington and at Millennium Business Center, which is 1.6 miles west of the new location.
Indicating that the new location will replace it, the NAI Hallmark commercial real estate firm is listing the 8,800-square-foot Conviva space at Millennium as “available soon” and divisible into two 4,400-square-foot spaces.
